The Medical Cardiology unit is a 29-bed unit consisting of 1 private room, 13 semi-private rooms and one 4 bedroom. Patients are received from the Emergency Department, Cardiac Catheterization Lab, CCU, MICU, physician offices, and other facilities when inpatient admission is necessary.

The patient population served includes male and female, acute and chronically ill patients with a variety of cardiac and medical diagnoses. Most patients admitted to the unit require telemetry monitoring after receiving minimally-invasive and invasive cardiac procedures. The age range is from young adult (18 years) to geriatric (100+) years.

Essential Duties & Responsibilities include but not limited to: 

  • Implements physician orders, administers medications, starts IVs, performs treatments, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations. 
  • Orders, interprets, and evaluates diagnostic tests to identify and assess patients' conditions.

The Emergency Department (ED) is a Level I Trauma Center and serves as a regional referral center for a wide variety of patients requiring acute care. Patients of all ages are seen 24 hours/day, 7 days/week.
There is a total of 45 rooms in the ED, which includes 5 barco-lounger spaces for lower acuity patients. The Emergency Department is divided into five major zones: Major trauma and critical care, Resuscitation and minor care, Pediatrics, Acute care, and Rapid Care.
There are two distinct waiting areas, one for pediatric patients and their families, and the other for all other visitors. There are 7 negative flow rooms, one of which is also an isolation room. Clinical and clerical workstations, nutrition areas, and supply and pharmaceutical dispensing machines are centrally located in each zone. A dedicated radiology suite, including 2 CTs, a DECONTAMINATION suite with a separate entrance, as well as a satellite lab are also located within the Emergency Department. In 2017, it is anticipated that Albany Medical Center will open a Level II Pediatric Emergency Department with 15 exam/treatment areas, 2 trauma resuscitation rooms, and a pediatric observation unit.
Each ambulatory patient is initially evaluated at the triage. Patients arriving via EMS are directed to the appropriate patient care room by the flow facilitator during peak hours. Acute psychiatric services are referred to the Psychiatric Crisis Unit at the Capital District Psychiatric Center. Acute burn patients may be transferred to a regional burn center after stabilization.

Minimum Qualifications: 
•    Graduate of a professional academic nursing program in which a Diploma, Associate Degree or Baccalaureate Degree is conferred 
•    Must hold current NYS Registered Nurse license 
•    Obtains and maintains certification in BLS/ACLS/PALS/TNCC 
•    Minimum of 1-3 years of previous clinical nursing experience in an Emergency Department, preferably in a large academic, Level 1 Trauma Center and Comprehensive Stroke Center

About the Role

Job Description

The Program Manager, Climate Solutions is a dynamic, engaged contributor to the overall efforts of Skyline Foundation, focusing on the issue areas pertinent to Skylines climate solutions program. The Program Managers core responsibilities include working on the execution of Skyline Foundations grantmaking in climate solutions with a commitment to trust-based philanthropy, equity, and building partnerships with grantees. The Program Manager will also contribute to grantmaking strategy by participating in continuous improvement within the program, analyzing and evaluating results to strengthen the climate solutions program strategy and impact.

Salary & Benefits

The salary for this position is $150,000 USD. Offers are based on the position's responsibilities, candidates experience, and Skylines practice of maintaining salary equity within the foundation.

The RN Admission Screening Coordinator provides oversight of pre-admission evidence-based criteria evaluations of non-transfer bed requests through identification of bed requests for non-transfer patients, review of established evidenced-based criteria, and adherence to established escalation processes to better ensure appropriate and timely reimbursement of these services rendered.

Essential Duties:

  • Collaboration and communication with key stakeholders including providers, managers, staff at clinics and hospital's ETC, Capacity Management, House Supervisors, hospital financial counselors, Utilization Management staff and Case Managers, Physician Advisors, and payers.
  • Robust collaboration with hospital financial counselors, Capacity Management, and House Supervisor regarding bed requests for non-transfer patients.
  • Evaluation of established evidence-based criteria for direct hospital admissions including, but not limited to: proposed admission status; status of authorization obtained by clinic staff and/or hospital financial counselors; appropriate level of care at admission.
  • Implementation of escalation processes, including communication with provider and consultation with Physician Advisor when criteria are not met and/or proposed admission status is incorrect, and the physician does not agree with RN Surgery Screening Coordinator determination.
  • Facilitation of scheduling of Peer-to-Peer meetings when payer denial received.
  • Documentation of evaluations, including escalations and outcomes, in the Electronic Medical Record (EMR) in a timely manner and viewable by key stakeholders such as Case Managers, Physician Advisors, Patient Access, hospital financial counselors, and Claims.
  • Post-admission validation of status ordered to confirm it is in accordance with established CMS and hospital guidelines including Medicare's Two Midnight Rule.
  • Collection of admission screening evaluation data for reporting, tracking, and trending of identified quality improvement opportunities in department processes, provider practices, and payer determinations.
  • Participation in department Inter-Rater Reliability process.
  • Maintains competencies for hospital, age specific and job specific standards of care; follows hospital procedures for accurate and timely processing of charges.
  • Represents the department in a positive and professional manner.
  • Supports orientation of new staff.
  • Supports hospital and departmental protocols, workflows, policies, procedures, standards, and competencies for clinical role.
  • Supports the mission, vision, and values of the organization.
  • Performs other duties as requested/assigned.

Required Qualifications:

  • Req Bachelor's Degree Nursing Degree from an accredited school of nursing
  • Req 3 years Experience as a Registered Nurse
  • Req 2 years Utilization Review and Case Management experience in an acute hospital setting
  • Req Experience with InterQual evidence-based criteria.
  • Req Demonstrates ability to utilize effective and appropriate oral and written communication skills.
