The compensation range for this position is $325,000 - $360,000. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The Physical Therapist is responsible for evaluating/assessing needs of patients and formulating treatment plans. S/he provides therapy services defined in treatment plans and works cooperatively with physicians. The Physical Therapist may supervise physical therapy assistant(s).

ESSENTIAL JOB FUNCTIONS

• Consults with patients to learn about their physical condition and symptoms.
• Provides manual therapy as part of treatment plan when indicated.
• Assesses and evaluates therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
• Instructs patients and families/caregivers in the use and care of therapeutic appliances.
• Determines priority needs for physical therapy.
• Reports to physician patient’s reaction to treatment or changes in condition and document in patient record (EHR).
• Initiates physical therapy program and coordinates with family members on home programs, exercise, and case conference.
• Identifies patient and family/caregiver needs for other home health services and refers as necessary.
• Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
• Performs other relevant job duties as required by manager/supervisor.
• Complies with federal and state regulations.
• Complies with APTA code of ethics.
• Complies with continuation education requirements.

QUALIFICATIONS

• Bachelor’s, Master’s, or Doctoral degree approved by the California Board of Physical Therapy.
• Licensed to practice as physical therapist in California.
• Must meet the educational requirements set by the California Board of Physical Therapy.
• 1 year of experience in outpatient setting.
• Current documentation of Basic Life Support is required.

LANGUAGE

• Must be able to fluently speak, read and write English.
• Fluent Chinese speaker (Cantonese and/or Mandarin) preferred.
• Fluent in other languages are an asset.
