Job Summary
Administers the nursing program in a long term care facility to maintain standards of resident care. Facilitates the optimization of the geriatric care process to improve the quality and efficiency of service. Responsible for the overall quality of care provided by the organization's nursing personnel. Advises medical staff, department heads, and administrators in matters related to nursing service and strategies. Interprets policies and objectives of nursing service to staff and community groups. Monitors the operations of the nursing staff and ensures compliance with regulations on organizational and governmental standards and practices. Schedules staff and conducts employee performance reviews. Provides nursing care to residents on an as needed basis. Assists in the development and implementation of marketing activities/programs inclusive of community activities, special events and media activities.
Has overall responsibility for the day-to-day operations of long term care facility. Includes quality of resident care, program/care management, environmental safety, budget constraints and responding to identified needs to ensure regulatory compliance. Performs necessary delegation and coordination for clinical, operational and managerial activities to accomplish the objectives of the department. Effectively manages and directs the work of individuals or teams.
Maintains communication and addresses the rapidly changing healthcare environment in which clinical services are delivered across the system. Leads the development and execution of clinical programs, policies, procedures and protocols ensuring system integration and adherence of clinical programs, policies, procedures and protocols ensuring system integration and adherence. Monitors operations of nursing staff and ensures compliance with regulations on organizational and governmental standards and practices.
Qualifications
Bachelor's degree in nursing preferred, but not required.
Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
For Rural Health Network facilities and Good Samaritan Society (GSS) locations only, employees who do not possess a Bachelor's degree, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor's degree within five years of hire into position. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring

Job Summary
The restorative nursing aide serves as caregiver to the resident during the scheduled work period in long term care. Uses appropriate mobility, balance and strengthening exercises, incorporating into therapeutic activities. Includes using adaptive equipment, fall prevention, appropriate turning, positioning and repositioning techniques, basic range of motion exercises, and good body mechanics. Assists with training and implementation of special programs designed to improve resident's quality of life, ability to perform daily activities and maintain their highest functional level of independence. Obtains resident's report prior to providing care. Documents resident's progress of care and behaviors, recognizes and reports changes in resident's condition promptly.
Includes dressing/undressing programs, grooming, bathing, toileting, exercise/movement, and eating/swallowing programs. Demonstrates correct use and application of ice and paraffin baths, heat packets etc. according to state specific regulations. Uses the EMR accurately. Handles linens properly and disposes of soiled linens, clothing or supplies in adherence to regulations and guidelines: uses standard precautions by using personal protective equipment and observes transmission guidelines, washes hands between resident cares, report signs/symptoms of resident infections to appropriate staff in a timely manner.
Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Considered a member of the nursing team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use.
Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16. Prior clinical or nursing assistant experience in long-term care preferred.
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing. Completion of a C.NA training program, training if required by state and at least 40 hours of specific training related to restorative care procedures (may be provided by licensed therapists providing service within the organization).
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor
vehicle report and proof of valid driver's license. Also must be medically qualified to perform the essential driving functions of
this position as per our Sanford Driving Policy per the leaders request.

Qualifications

Responsibilities: Under general supervision and medical direction performs respiratory care related procedures. Exercises considerable judgment in accordance with physician's treatment plan and within the scope of departmental policies and procedures. Scope of practice includes patient/family assessment and management, resource management, care facilitation, and discharge planning as required. Observe and chart: pulse, respiration, breath sounds (number and quality), and resident's general appearance, including breathing exercises and teaching and training to residents.
Notify supervisor of all new starts and discontinued residents. Administers proper technical respiratory care procedures. Dispenses proper dosage of medication, acquires and adjusts devices to insure optimal results from therapy. This includes mask, mouthpiece, or Trach equipment for resident's comfort and ease of operation, regulates flow and pressure of equipment. Observes resident's respiratory rate and pattern, pulse, breath sounds, reaction to medication, any difficulty with therapy during and after treatment. Chart and communicate to the resident's nurse any pertinent information. Observe thorough and accurate charting technique. Inspects and tests equipment, disassembles, cleans, reassembles equipment as required. Maintains adequate supply of necessary equipment. Performs administrative duties and record keeping when required. Disposes of equipment properly. Assists in emergency cardio-pulmonary resuscitation as an integral part of the emergency team. Instructs residents and family members in the rationale and methodology of every respiratory procedure administered to each resident to facilitate resident cooperation, understanding and efficiency of treatment. Job Summary
The restorative nursing aide serves as caregiver to the resident during the scheduled work period in long term care. Uses appropriate mobility, balance and strengthening exercises, incorporating into therapeutic activities. Includes using adaptive equipment, fall prevention, appropriate turning, positioning and repositioning techniques, basic range of motion exercises, and good body mechanics. Assists with training and implementation of special programs designed to improve resident's quality of life, ability to perform daily activities and maintain their highest functional level of independence. Obtains resident's report prior to providing care. Documents resident's progress of care and behaviors, recognizes and reports changes in resident's condition promptly.
Includes dressing/undressing programs, grooming, bathing, toileting, exercise/movement, and eating/swallowing programs. Demonstrates correct use and application of ice and paraffin baths, heat packets etc. according to state specific regulations. Uses the EMR accurately. Handles linens properly and disposes of soiled linens, clothing or supplies in adherence to regulations and guidelines: uses standard precautions by using personal protective equipment and observes transmission guidelines, washes hands between resident cares, report signs/symptoms of resident infections to appropriate staff in a timely manner.
Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Considered a member of the nursing team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use.
Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16. Prior clinical or nursing assistant experience in long-term care preferred.
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing. Completion of a C.NA training program, training if required by state and at least 40 hours of specific training related to restorative care procedures (may be provided by licensed therapists providing service within the organization).
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year.

    The Primary Care Physician (PCP) in our organization demonstrates:

Each PCP will have goals for these metrics and will be expected to work towards those targets with their center and market leadership as well as their care teams.     The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals.     The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures. ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
    • The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
    • The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
    • It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
    • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
    • The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
    • For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
    • PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
    • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
    • Performs other duties as assigned and modified at manager's discretion.

    PAY RANGE:

Our Respiratory Department is dedicated to providing quality, patient centered care and service to the citizens of this community.     Some facts about us:

    • We utilize Respiratory Driven Protocols to manage patients in ICU and acute care areas. After completing competency, RTs are able to intubate and insert A-lines
    • We work in adult cardiac care unit (ICU), medical/surgical ICU, progressive care unit (PCU), acute care floors (womens, medical, surgical, joint/spine, neuro)
    • We have a Womens Center with a level 3 NICU. RTs will attend deliveries and care for babies who are admitted to the NICU receiving respiratory care
    • We have a pulmonary function lab (PFT) where RTs perform outpatient and inpatient PFTS
    • We perform inpatient EKGs on the general care floors, occasionally outpatient (shared service with cardiology)
    • We manage GE Carescape R860 vents, Servo I vents, Drager VN500 and HFOV NICU vents, Phillips V60, Bubble CPAP, REMstar CPAP, Smart Vest, and Airvo
    • We perform bronchoscopy assistance with Glidescope disposable bronchs
    • Opportunity to cross-train on EEGs, PFTs and NICU
    • Option to participate in Annual Pulmonary Conference, Leadership Development classes, and Skills Development Courses
    • Our team consists of approximately 50 members, 1 director, 1 supervisor, 1 NICU lead, and 2 adult lead
    • We typically run 7 people during the day and 6-7 at night. Varies per census and acuity
      • We cover our South Campus which has an ED, behavioral health unit and acute care rehab. 1 RT for each shift.
    • We allow candidates to shadow our department and facility
    Responsibilities:

    RESPONSIBLE FOR:
    Performing patient assessments, general respiratory care procedures and protocols, critical care procedures in adult care areas, blood gas analysis, maintenance and management of all equipment and patient education.

    Qualifications:
