Join True Health NY

True Health NY is seeking a dependable, clinically strong and patient-centered Primary Care Physician to serve as the physician anchor for our Williston Park clinic.

We welcome physicians trained in either Internal Medicine or Family Medicine who are committed to comprehensive adult outpatient care. The successful physician will establish and maintain a stable patient panel, improve access to primary care, manage acute and chronic medical conditions and provide consistent clinical leadership at the Williston Park location.

The Williston Park clinic provides an integrated outpatient environment with adult primary care, OB/GYN, women’s health, ultrasound and coordinated diagnostic and administrative support.

This is a site-anchored position—not a four-office float role. The physician will have a defined Williston Park schedule, dedicated clinical support and clear responsibility for patient continuity, documentation and clinical follow-through.

Position Highlights

  • Full-time outpatient primary-care position
  • Defined Williston Park home office
  • Four- or five-day schedule options
  • No routine inpatient responsibilities or hospital call
  • Established demand for adult primary care
  • Opportunity to build and retain a long-term patient panel
  • Dedicated medical-assistant and administrative support
  • Centralized scheduling, referral, authorization and billing support
  • Access to OB/GYN, women’s health, ultrasound and diagnostic services
  • eClinicalWorks electronic medical record
  • Competitive guaranteed base salary
  • Written productivity and quality incentive
  • Potential sign-on and relocation assistance
  • Opportunity for future clinical and primary-care leadership

Clinical Responsibilities

The Primary Care Physician will:

  • Provide comprehensive adult outpatient primary care.
  • Conduct new-patient examinations, annual physicals, preventive visits and follow-up care.
  • Provide Medicare annual wellness visits when appropriate.
  • Offer same-day and acute-care appointments when clinically appropriate.
  • Diagnose and manage common acute and chronic medical conditions.
  • Manage hypertension, diabetes, hyperlipidemia, obesity, thyroid disease, asthma, COPD, cardiovascular risk factors and other common adult health needs.
  • Provide medication management and medication reconciliation.
  • Conduct preoperative medical evaluations and appropriate medical risk assessment within scope.
  • Provide evidence-based screening, immunization and preventive-care counseling.
  • Order and interpret laboratory, imaging and diagnostic testing.
  • Review and communicate clinically significant findings promptly.
  • Develop clear treatment, monitoring and follow-up plans.
  • Coordinate referrals and consultation with specialists.
  • Maintain continuity for medically complex and high-risk patients.
  • Identify conditions requiring emergency, hospital or specialty escalation.
  • Provide telehealth services when clinically appropriate and operationally approved.

  • Post-hospital and emergency-department follow-up
  • Medication reconciliation
  • Care-gap outreach
  • Appropriate referrals and follow-through

Clinical services must remain evidence-based, medically necessary and within the physician’s training and scope.

Integrated Care Responsibilities

The physician will work collaboratively with OB/GYN and women’s-health clinicians to:
