Job Description

Job Description 

This individual is responsible for supporting the multidisciplinary care and coordination of patients with PH across the inpatient and outpatient settings while helping ensure ongoing compliance with PHA accreditation standards and best practices.

The PH Nurse Navigator functions as a central liaison between patients, physicians, advanced practice providers, specialty pharmacies, nursing staff, respiratory therapists, pharmaceutical representatives, hospital departments, and external referring providers. JOB SUMMARY

The Hartford HealthCare Heart & Vascular Institute is seeking a Physician Assistant or Nurse Practitioner (APRN) leader in Cardiology at Backus Hospital as well as supporting the outpatient Hartford Healthcare Medical Group office setting to coordinate an advanced level of comprehensive health care for cardiology patients. This role specifically oversees the work of the APP team in cardiology supporting the Norwich provider group of general cardiologists and non-invasive procedural cardiologists and takes a leadership role in overall program development and quality.

Position Highlights

As an Urgent Care Advanced Practice Provider, you will work collaboratively with experienced physicians and APP colleagues while practicing the full scope of your training and expertise.
