JOB SUMMARY Position serves as an integral part of Hospital management providing nurse supervision and direction as well as administrative support.

WORK ENVIRONMENT Work is performed in a hospital, addiction rehabilitation program and/or outpatient medical services environment. There is common exposure to contagious disease. The work may include a certain amount of exposure to hostile and emotionally disturbed patients, families and visitors. Special attire may be required as protection against infection.

PHYSICAL DEMANDS This position requires the demands of long periods of standing and walking. Working with patients requires recurring bending, lifting, squatting, stretching and positioning of patients. Must be able to lift up to 50 pounds. Must manage stress under many different circumstances.

ESSENTIAL FUNCTIONS Satisfactory job performance will be determined by successful execution of the following: A. Maintains RN core competencies. B. Consults other departments, as appropriate, to provide for an interdisciplinary approach to the patient's needs. Treats patients and their families with respect and dignity. Identifies and addresses cultural and psycho-social needs of patients and their families. C. Facilitates and/or participates in patient group therapy and encourages patient participation. D. Ensures patient home medications are properly verified and entered into the EHR E. Educates staff on patient's medication, medical, and/or physiological needs F. Assists in the orientation of new patients into the rehabilitation program. G. Assists in the monitoring and documentation of patient participation, direction, behaviors, compliance with schedule and rules, and relapse prevention. H. Facilitation and monitoring UA's I. Coordinate with program managers to ensure patient care plans are met. J. Assists the Program Director and Medical Director in revision, implementation and development of policies and procedures that support best practice and are in line with regulatory requirements. K. Coordinates and directs patient care and education by auditing care plans and assisting staff members to improve the quality of care rendered. L. Recommends the purchase of supplies and equipment required for the program. Ensuring adequate supply levels are maintained. M. Routinely checks SQSS and maintains regulatory reporting compliance data. N. Assists program director in maintaining staffing levels to meet the care and treatment needs for all patients. O. Assists program manager with time keeping and monitors staff attendance. P. Investigates and reports patient complaints to the program manager and/or hospital administrator. Understands and follows the MCNDH grievance policy and procedure. Q. Maintains open communication in person and distant to resolve any pressing issues or events. Reports to Program Director, Rehab Medical Director, CNO and/or Hospital Administrator any issues that rise to that level of concern. R. Maintains a professional demeanor and promotes professional, ethical and legal standards of nursing. S. Regular attendance and professional attire required. T. Performs other duties as assigned.

Qualifications

MINIMUM QUALIFICATIONS

  • Education Incumbent must be a graduate of an accredited professional school of nursing. BSN preferred.
  • Experience Prefer one year of supervisory experience.
  • Licenses & Certification Incumbent is required to possess current licensure by the State of Oklahoma or National Council of State Boards of Nursing for Registered Nurse. BLS, ACLS, and CPI, or equivalent de-escalation training.

Knowledge & Skills

  • Professional knowledge of nursing care principles specific to behavioral health and recovery of addiction. Incumbent must also have knowledge of quality management processes, ethics, and infection prevention practices
  • Specific knowledge of psychiatric nursing disease and the disease process.
  • Knowledge of pharmaceuticals, their desired effects, side effects and complications of their use.
  • Ability to recognize and react to emergency situations both medical and psychiatric.
  • Knowledge and skill in operation and monitoring of general nursing equipment.
  • Ability to develop, implement and evaluate a patient care plan.
  • Computer literacy and ability to become proficient in electronic medical record.
  • Ability to communicate professionally and effectively in oral and written form.

Scheduling & coordination (10%)

Plan and schedule field visits with district teams while reviewing drawings, equipment lists, and job requirements in advance. Coordinate logistics and provide pre-delivery operations worksheets and daily work schedules so customers understand assembly methods, sequencing, labor needs, and required tools before equipment arrives on site.

Reporting, safety talks, and lunch & learns (10%)

Complete field service fact sheets promptly, documenting site conditions, operational observations, customer feedback, safety concerns, and recommendations to support continuous improvement, customer satisfaction, and sales efforts. Lead and facilitate training initiatives to reinforce safe handling and efficient use of EFCO systems, including toolbox talks, safety talks, lunch and learns, and other knowledge-sharing sessions with district teams on best practices and operational insights.

Foster Care Case Manager

Foster Care Case Managers play a critical role in supporting children and families within the child welfare system. They begin working with children following a court order that removes the child from their home and continue providing support throughout the child's time in foster care.

In this role, case managers work directly with children, parents, and caregivers while also leading a collaborative, multidisciplinary team focused on ensuring the child's ongoing safety, well-being, and stability. Their ultimate goal is to help each child achieve permanencywhether through safe reunification with a parent, adoption, guardianship, or preparation for independent living.

This highly impactful position requires a unique combination of skills and personal strengths, including compassion, strong organizational abilities, effective communication, and perseverance. While foster care case managers receive regular supervisory support, they must also be confident in making independent decisions, conducting ongoing assessments, and adapting creatively to a wide range of complex and evolving situations.

Additionally, this role requires flexibility and a strong commitment to meeting the needs of children and families. Case managers often adjust their schedules to connect with clients in community settings at times that are most convenient and supportive for them.

Core Responsibilities of a Foster Care Case Manager include:

  • Monthly home visits with foster children: while the frequency can vary by case situation, all foster children on your caseload must be visited at least monthly in their residence (e.g. foster home).
  • Monthly visits with parents of foster children: since the goal for all foster children is reunification (until a judge will order otherwise), this requires monthly visits and additional communication with the child's parents. You will seek to engage them in treatment plans that help them succeed in reunification with their children.
  • Timely report writing: About half of the case manager's job involves computer-based paperwork, most important of which is the quarterly report on each foster child and their family, written for several key readers including the judge and attorneys assigned to the case. So, it's critical that case managers can communicate effectively in writing and that they are able to do so in a timely manner.
  • Court testimony: Since all foster children are in the foster care system as a result of a judge's order, that means that the court has an open Abuse/Neglect case on the children and their families; and that means that the court requires regular (normally quarterly) court appearances by the parents and the case manager to update the court on progress.

Essential Responsibilities:

  • Commitment to learn and comply with MDHHS foster care policies and procedures as outlined in the Foster Care Online Manual
  • Commitment to learn and comply with applicable rules and regulations from Department of Child Welfare Licensing (DCWL).
  • Timely completion of computer-based and paperwork reporting.
  • The Foster Care Case Manager should expect a caseload of 13 children.
  • Cooperate and assist other agency staff (i.e., licensing, medical and adoption personnel) as well as partner agencies with necessary paperwork requirements.
  • Comply with agency expectations related to general forms (i.e., expense vouchers, service area reports, etc.).
  • Coordinate and monitor community resources available to clients (i.e., school conferences, psychological/psychiatric evaluations, parenting classes, substance use disorder, sexual abuse and mental health therapy and medical services, etc.).
  • Attend agency and team meetings, case conferences, adoption planning conferences and in-service training, etc.
  • Develop and implement initial and updated service plans, parent-agency agreements, aftercare treatment plans, and counsel parents in the completion of these plans.
  • Participate in required case-related activities such as testifying at court hearings, pre-hearing conferences and Family Team Meetings (FTM), and Foster Care Review Board meetings.
  • Safely transport youth.
  • Must be available at night and weekends, in addition to our on-call team.
  • Other responsibilities shall include those set forth in agency personnel policies or as assigned by the supervisor, management, and leadership.
  • The ability to work more than 40 hours per week to achieve Agency objectives on a timely basis.

Supervision Received: General Direction- Plans and arranges own work. Uses a wide range of procedures to accomplish assigned objectives.

Supervisory Responsibilities: No supervisory responsibilities.

Minimum Qualifications:

  • Educational requirements are a minimum of a Bachelor's Degree in Social Work or a closely related Human Services field as approved by the Bureau of Children and Adult Licensing (BCAL). Master's degree is a bonus.
  • Positive acceptance of supervision to learn and be trained in case management, assessment and treatment skills.
  • Commitment to providing excellent child welfare services.
  • Child welfare experience is a plus.
  • Experience or training in working with children or adults within individual and family counseling, interviewing, family assessment and crisis intervention.
  • Valid driver's license, good driving record and adequate insurance with a reliable vehicle always available.
  • Possess an understanding and sensitivity to the client's cultural and socioeconomic characteristics with personal qualities of warmth, sensitivity, and the ability to establish meaningful relationships with people from various cultures.

JOB SUMMARY

In this role, you will be responsible for ensuring safe non-medical transportation for our clients. You will be expected to provide quality support services to clients served by Buckeye Residential Solutions. You will also be expected to maintain accurate records as required by Buckeye Residential Solutions and any other governing agency such as DODD, ODHS, and County Case Management.

PRIMARY RESPONSIBILITIES

  • Interacts physically with each individual and co-worker in a socially and professionally acceptable manner.
  • Maintain a clean, safe, and healthy environment.
  • Carry out individual service plans (ISP), especially those specific to transportation.
  • Ensure the appropriate forms are in the vehicle at all times.
  • Verify there is a first aid kit and fire extinguisher in the van at all times.
  • Sign off on all required documents in regards to the client for which they are

providing services.

  • Complete required documentation including but not limited to: van inspections, NMT transportation sheets, and incident reports.
  • Reports any client issues to the Program Coordinator.
  • When transporting the clients:
    1. Complete a visual check of the client to ensure clean/ appropriate clothes for the weather and hygiene.
    2. Ensure all clients are wearing their seatbelt.
    3. Understand and obey all traffic laws
    4. Ensure that all consumers disembark and board the vehicle safely with physical assistance if and when needed.
  • Attend and participate in scheduled staff meetings, designated training and in-service sessions.
  • Adhere to confidentiality guidelines, as outlined by organization policy and HIPPA regulations.

Other Duties as assigned.

WORKING CONDITIONS:

  • While we make every effort to accommodate your schedule needs, hours are based on the client needs.
  • Job will require sitting, standing, bending, lifting, pushing, and pulling, intermittently.

QUALIFICATIONS:

  • No job-related experience necessary!
  • 18 years or older.
  • High school diploma or equivalent.
  • Current certifications in: CPR, CPI, MUI, First Aid, Medication Administration, and Documentation Training (We can train/ provide these for you).
  • Must possess a valid, current driver’s license.
  • Valid proof of auto insurance.
  • Ability to complete and pass a background check, drug screen, and physical per company standards.
  • Positive attitude with the desire to help others.
