Physical Therapist - Digital Health Specialist - Hybrid Role

This is a hybrid position working 3 days in person at our CORA clinic and two days working remotely as a Digital Health Specialist. The Digital Health Specialist (DHS) provides digital health services, including virtual care and remote therapeutic monitoring. The DHS participates in the development of the digital health program and serves as a liaison between patients and in-clinic treating providers. This is a full-time position (40 hours/week, 5 days/week) (40 hours/week, 5 days/week). Must be licensed in any compact state. Must also be willing to obtain licensure in other states.

Job Summary

This role floats and provides coverage at Urgent Care offices in Stamford, Norwalk and Wilton bringing variety to your workday and the chance to collaborate across multiple teams and communities. Reliable transportation needed. 

Under the supervision of a Registered Nurse, or Provider, the LPN provides quality and cost-effective direct care for patients/families within a unit, service or other primary area of practice.

 

• Collaborates with members of health care team to assist patients in achieving established outcomes of care
• Participates in direct patient care in order to meet individual and family health goals by: obtaining, documenting and reporting patient data related to established plan of care
• Performs therapeutic measures, under the direction of an RN or MD, to assist in achieving established patient care outcomes.

Job Description

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. • Reviews patient records, including preoperative records, to ensure completeness and reviews outstanding issues with provider
• Demonstrates independent nursing actions based on assessment and problem identification
• Under direction of provider, calls in prescription medications to pharmacies and observes and documents patient’s response to pertinent medications
• Documents accurate and ongoing assessment of patient’s needs, problems, capabilities, limitations, progress toward goals, and response to care
• Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner
